It now features the ability to be filled out on your computer, which also allows you to submit the form by email. We implemented these features as a convenience to you and to assist us in processing new orders. Please be aware that the most recent version of the free Adobe Acrobat Reader is recommended to fully utilize the form's features. The updated form can still be filled out in the usual manner and be faxed or mailed to us.
